How would you get this effect?

how about just using gradient and multiplying / darkening out the dark areas? just an idea.

It's cross processed

Originally a film developing process, but can now be achieved through altering the colour channels in Photoshop.

A goggle search for the term should give you some tutorials.

I think the photograph used 1 external flash. Then he altered the colour channels.

The flash was on the camera, which is why it looks so crummy.
