Is it my lens, my camera or me?

As usayit said, if it was shot at F/22 it will be soft, I don't think any lenses are razor sharp at F/22
Lots of lenses are the sharpest at about F/5.6 - F/8

Shot at F/22 at 1/3 sec. Ya, it was pretty bright out, you need a nice ND filter to use a bigger aperture... F/22 is too small, I never go that small...

Oh yeh... Teslahorn did bring up something that I completely forgot (I never forget if I"m actually looking and setting up my camera).

With really slow shutter speeds

1) Heavy tripod (or hang something heavy underneath).
2) Use a wired remote or timer function (avoid touch the camera).
3) Leverage mirror lock up ( I believe your camera has that feature).
 
Always use a remote trigger (sorry if I failed to mention that) if I am shooting waterfalls. The Polarizer is the top end hoya. I will try stepping back the aperture and see what I think. I always thought that you wanted to go with a 22 range to get as much as you could in focus front to back... Will look into learning how to do the mirror lock up to see if that helps, and also hanging something heavy from the tripod...
