legal photo release??

To make a long story short, I'm trying to start a photography business. I already have a few photo shoots lined up to get a portfolio started and some experience under my belt, but I know that I am going to need a photo release form. Something that just states that I own the photos and have permission to sell them, or alter them, and claim them as my own. The client will have the photos on a cd, but are not allowed to claim them, but they do have printing rights. Is there anybody that could help me write something out, or point me in the right direction to getting something written? And is there anything else that would really need to be included?

btw, I'm new here. My name is Jessica, and have always been interested in photography. The last couple years it's turned into a passion, and my 9-month old is now my inspiration for FINALLY starting a photography business of being a photographer for infants and children.
