I bought a WB card but I can't fill the frame with it when I use my 135mm lens and it be in focus.

It's not really all that important for it to be in focus nor to fill the frame. I generally fan them out and either lay them in the shot or have the subject hold them on the first shot. They are not necessarily the answer to all WB setting though, as I found out recently. Sometimes the light is so far out there, that reflections on the cards can throw the eye dropper tool in LR off. That's when you have to use your eyes and a calibrated monitor to manually adjust for a pleasing WB.

OP: There may be something else going on - I was able to get things a bit warmer but not as much as I'd have liked.

I like your edit. It's pretty close. I took a quick try at it in PS with an adjustments in levels, brought the red down slightly, and cleaned up the skin tones, little dodging and burning and
