Need help with B&W photo

1.I want to know if it is possible to make a photo like this alone or if i need someone else to hold the camera behind my head.

2.I also want to know if it is possible that the person who made the image made it alone considering that it was made with the iphone

maybe just extend your arm out, select the front facing camera, and shoot. Considering the angle, it would go with someone hand holding it at an angle which is how the body is anyways.

I really don't see what the problem is here other than it's totally underexposed.

Looks like an accidental shot when someone set their phone down on the counter in a restroom.
Why would you want to do this on purpose?
