I shot some photos for a musician this evening...

I need a release or license form so his agency can use the images to promote his music.
I do not want to surrender my copyright of the images. I plan to use them for myself as well.

Does anyone have a copy of such a release or a source for such forms?

I already have a model release from the musician. I need a release that I can give to his agency that will grant them license to use the photos I took.

Remind me to let you look through the disc I have of forms. I think there is a release of what you're looking for.

You can just write (type) out a release can be very simple "I <your name> allow <his agency> permission to use X photos for promotion of <artist's name> career for X amount of time. This rights release does not include permission to sell said images in form.

Sign and date with a witness signature and preferably notarized.
Also if possible have his agent sign a copy for you to keep. No not if possible sorry make sure his agent signs.
