Does anybody know if the Extreme USB 2.0 Reader will work with Extreme IVs? I've read that it is not UDMA compatible so does that mean it won't work at all or just not at those speeds?

Backwards compatibility. There's many standards before DMA was created which work just the same way except oodles slower.

Although I don't quite now how it would possibly apply to card readers and cards. Any CF card reader can read any CF card.

It'll read the card, but usually only the FireWire card readers will read/write the UDMA equipped cards at their full speed. FireWire is uncompressed while USB can transfer compressed files at 480mb/s, it's usually sustained at 20mb/s. UDMA cards are faster than that so although they can be read by the USB equipped drives, it'll be at 20mb/s, not the higher speeds.
