I'm only going to list the odd things I have. I should mention that recently a friend cut her finger and I whipped out my antibiotic cream and bandaids. "My god, do you carry everything in there?" Pretty much.

So, my photo stuff that you didn't list would be extra SD cards, one spare battery fully-charged, a lens pen--but no lens cleaning kit--and a remote control. I carry a beanbag and occasionally use the beanbag and the remote control stable platform. When I taking photos of kids we will play with the bean bag as an icebreaker sometimes.

Are all the portable ancillaries to be included?

... I go to The Gambia soon for ...
I had a P&S when I went. I think the Ex took the images along with everything else when she left. The cows on the beach cracked me up. A student riot happened on the day of arrival, so the first few days were to stay on the compound. Very gracious locals, but rife with poverty back then, 1999 or 2000.

One camera, a tripod and two lenses max
 You know to keep everything close at hand, I'm sure.

+1

Yeah, on a vacation? Keep it simple. I have a little backpack bag I take on vacation and I generally carry one camera, one very wide range zoom and either a much longer-range zoom or a much wider lens, depending on what kind of pictures I expect to take. I also bring my portable tripod and not my big manfrotto. I try to squeeze the external flash in theres somewhere if I can, but... Honestly, even this much equipment is kind of annoying on a trip.

Looks like I've got the most basic bag so far. My Lowepro Nova 1 AW contains:

Canon Rebel XTi packed with a 2GB Sandisk CF
Canon 18-55 f/3.5-5.6
Sigma 70-300 f/4-5.6
Raynox DCR-250 macro lens
Hoya CPL
IR remote
a microfiber cloth

That's it, pretty much everything I have right now, my tripod won't fit the small bag.

i don't bring a bag, i put my lens on my pocket and the camera in hand.
50mm on my right pocket together with 4cf cards and 70-300mm on left, 18-70 on the camera. bag bothers me..

hmm for my bag:

Canon 400D+battery grip
400D kit lens (my widest lens ATM)
Canon 70-200mm f2.8
Sigma 150mm f2.8 macro
Canon+Sigma 1.4TC
Canon+Sigma 2*TC
Focusing rail
055XPOB Tripod
322RC2 head
screwdriver (for the tripod head)
microfibre cloth
waterbottle (outside pouch)
lens cleaning spray+wipes
lens brush
Lumiquest softbox
Speedlite 580M2 flash
2*2GB memory cards
2*4GB memory cards (one in camera)
remote cable release for 400D

And that is it - still very much building my kit up - teleconverters will probably get removed as I pick up some more lenses ;)

For a holidy the above is what I would take - my macro (as I love macro), a good zoom lens (though a little short its good for general stuff to a degree) and my kit lens for landscapes and general walking around with family (anything longer like the 70-200mm can get to be too long - especially on crop sensor) when out with family and you don't have space/time to move back
