Autofoucs vs manual focus settings

I have a D90 on which I generally use an AF-S Nikkor 18-200 G ED lens.
The lens has a MA-M toggle and the camera body has a AF-M toggle.
I use aperature priority mode almost exclusively.
When using autofoucs I set the toggles on MA (lens) & AF (body)
When using manual focus I set the toggles to M & M
Is this correct?
 
You only need to set the lens to M to focus manually.
 
Some lenses use a focus motor in the camera body to focus, so if you have such a lens you would need to turn the AF button on the camera to OFF.

To tell if a lens uses (actually, requires) a body motor to focus, look at the rear of the lens. Hold it so the contacts are at 12:00 o'clock. If there's what looks like a straight-slot machine screw at the 4:30 position, that's the connection to the camera's focus motor. You can actually take a jeweler's screwdriver and turn it and the focus ring will turn.

Not all Nikon bodies have this internal motor, so even "AF" lenses that require it will not auto-focus on a body that does not have that internal motor. If you use a lens like this, you would need to turn off the AF on the camera body.
