glossy/glass eyes

hows does one achieve the very glossy/glass like eye appearence in portraits?
 
It's important that you are accurately focused on the eyes, and that you get a catchlight (the lightsource is reflected in the eyes).
 
I find that a little dab of unsharp mask makes sparkly things sparkle a little more as well. As ksmattfish says, good lighting is essential, there must be an unbroken path from the light source, bouncing off the eye and to the camera to give a highlight (or catchlight). A stronger light source makes it easier for you or the camera to focus accurately as well.
