Model Release for Book and Gallery

Hi everyone. I'm working on a photo series which will involve various non-professional models. My intent is to use the images not only for my portfolio, but in an online gallery and eventually a book (long term goal). Because I eventually want to use these images in a book, I realize I need the models to sign a model release - Can you tell me what kind? Is there a special release for print or books? And if so, where can I find one? Are there any online?
