modeling releases?

If this is so important, why don't I see all studios and photogs requiring everyone they take a picture of, to sign a form like this?

Or is this just for people who want to be labeled "models", for which their images could generate a significant income for the photog for commercial purposes?

Some photographers will only get a release from someone whose pictures they want to use in their portfolio or for advertising.
