Personalized Photo Order Forms and online pre-ordering website options?

Hello all, I am new here but plan to be involved. Seems like a great forum. I have been doing photography for 17+ years and now have just recently broke free from the corporate world and have started my own business doing photography among a few other services.

I have a question for those who are already out there.

1. Where can I get the personalized photo order forms (mainly for sports portraits) printed, the ones that can also be sealed with money after the order, but in color and with my pricing?
2. What sites do you use to have clients pre-order images?
3. Lastly, what companies do you use for printing plaques, player cards, etc. (anything out of the normal of 8x10, 6x9, 5x7, 4x6)

Any help would be appreciated.
