Photo Release?

I was called when a friend trying to get pictures printed off at Sams Photo Center that I need to provide a Photo release document. I have asked around and have been told that when I hand off the cd of pictures I should also hand off a photo release document so they can publish them. I never knew this. (JUST STARTED) Can anyone direct me to a generic photo release document or a sample of one?
 
I have photo release forms. I can get one up here for you within the next few days when I get some more time. I was advised to get some by a friend of the family who is a pro photographer since I am a Fire Photographer.

I don't get why they need a photo release. I've had photos done by a few different photographers and have never needed a photo release.
 
Sams Photo Center locations are in Sam's Club and are part of Wal Mart. They require releases. I had a relative that had the same problem at a Wal Mart and was told the same thing. I checked with the family friend who is a Pro Photographer and he confirmed a lot of places are now doing this so they won't get sued with the amount of people who are sue happy now these days.

I print at wal-mart and have never had an issue.
 
It's not just a case of sue happy culture - its a direct move to protect the legal rights of professional photographers in the digital age; when printing is a very easily accessed resource. Generally speaking its hit and miss if they demand a photo release and its normally based on a guess by teh techs/admin as to if the photos submitted for printing are over a certain level of quality (ie not your average mum and dad shots of the kids).

There are many who've had to produce some kind of proof when they go to collect their prints from shops because the quality is higher than expected for the average person.
 
I have had Walmart ask for them before.. if the photo LOOKS professionally done then the associates are SUPPOSED to ask for the photo release and if you dont have one they arent supposed to sell to you. I printed off my family pictures I had professionally done at Walgreens, and they never asked for the release.

So if there is a watermark they are going to ask for one?
 
watermark, they should be asking FOR sure. But again its really those who play by the rules and those who dont. :)

That is a model release, and not a very good one.

The OP is asking about a Use license, or Print release (Modify to suit your needs):

Print Release - Use License

All images © 2011 [name or studio/business name here], All Rights Reserved. This Use License shall be governed by the laws of the State of _________.
I, [name or studio/business name here], as copyright owner of these images, grant a lifetime, personal, non-commercial use license to ____________________and their immediate family to print or have printed (no larger than(whatever size, if any) reproductions of these images for display in their home and workplace only.

Online Use
[Name or studio/business name here] has provided web sized and watermarked images for exclusive use on social networking web sites that are not owned by ____________________ and their immediate family. No other online use is granted. Removal of the watermark from these web sized images, or any other violation of any of the other terms, will constitute a breach of this entire Print Release – Use License, rendering it null and void in its entirety.

Copyright Information
Please remember that because these images are protected by Federal Copyright laws they may not be altered, copied, transmitted or used in any way not stipulated above without prior written consent of the copyright owner, {name or studio here}.

I will post a copy of my release form this weekend if you would still like since someone else already put one up. My release forms are a little more in depth than the one shown above. If water marks are on pics, photo stores are supposed to ask for release forms before making copies. Some places require it.

I have use licenses that run to several single space typed pages, but those are for commercial customers and would not have been appropriate for what the OP needed here.
