I find this to be a very sad picture. You capture very well the (sorry for the crassness, but) superficial and ugly nature of this old woman.

I still find it hard to look away, especially the woman with the doll, which adds a droll humor that really elevates the shot. I'm glad you titled this "doll", both for the dual meaning but also to draw our attention to that point.
