Except I made the photograph part of the image with a 50mm prime. The rest is various kinds of Photoshop trickery, in part known as Magic Circles.
Magic Circles are made using the Polar Coordinate filter in Photoshop.
Flickr has them all over the place. Just enter 'magic circle' in the Flickr search box or google it.
