How was this achieved?

? A whole lot of things. You need to be much more specific if you only want to know about one part of it. "Just the effects" doesn't really mean anything. Do you want advice about the lighting or possible post work or both, posing, what?
 
It looks to me like someone used two layers with the same photograph on each layer.

One of the layers then had the black point raised (or a curves adjustment to accomplish the same thing) to create that "soft/light/misty" look and was perhaps blurred a bit.

Then layer masking was used to blend the misty layer and the unmodified layer together, so the girl comes out sharp and high contrast, with dark blacks in the middle of the frame, but everything around her fades into the misty/blurry mess.
