Noise Problem

Is there a way to fix this level of noise? I've tried Neat Image but I can't get it right. Is it time to give up?

I suspect that it could be significantly improved in an advanced editing program like Photoshop. I can have a go for you and tell you how I did it if you like?

There's actually still some noise artifacts there, but I couldn't be arsed to do a proper job unless you like it and you want me to.... then I'll do it for you gladly

Just tip, it is really much easier to critique if you shrink the image down so we dont have to scroll alot to look at the whole thing. Just so we an take the whole image in at once.
