order forms

Hi, I am new to this forum and found you guys by googling. I am an event, sport and daycare photographer. I recently have quit my daycare freelance job and will go out on my own. I just need some help here and hope that someone(s) in this forum can help me. I am in the process of designing my order forms and would like some imput from season daycare or school photographers. Layout, what to offer, price range, view some order forms, etc.
If I could get some ideas as currently my brain is DEAD from other work I have had to do, I would greatly appreciate it.:hail:
