I was once forced to use a 50 pound motorized longroll cam with tethered remote to elevate and tilt the camera 5/8.... The O58b auto tripod is like Nirvana.

I find that the best shots usually come towards the latter part of the session when the model has had a chance to get used to the camera and is feeling more relaxed. THEN comes the work ... dump, dump, dump, dump, dump! Someone here told me several years ago that the difference between a good photographer and a bad photographer is the size of their wastebasket.

Yes, I'm finding this to be true. I don't know of any other way but I think I have to coach the client up front on the pose and jaw line, eye squench (Peter Hurley) and this causes them to think about what they are doing. Not so good but as we get going they tend to relax and find it not so difficult and then they take over their own expressions which is what I want. And then shoot, shoot, shoot, as you say.
