Appointment Template in Outlook

I can't find any info on the google-net on this subject at all... which leads me to think it's not possible. But here's what I want to do:

I need a template for appointments in my outlook.
Basically people book my time by going to my shared calendar in outlook and then entering an appointment in there.

Problem is they ALWAYS forget some important detail... like what the photos of, or where I'm meant to be going, or their own phone number.
So I thought if when they double clicked to enter an appointment it could come up with a template with stuff like this in the notes section:

Location:
Contact Name:
Description of Photo Required:
Ad number:
Rep Name:
Rep Phone Number:


Anyone know if this is possible?
