Canon Manual Diaphragm Adapter

I put this in the Buy/Sell first but thought it might be better here.

I have looked everywhere for the 35mm film can that contained my adapter. Can't find it anywhere and can't seem to find one on the net anywhere. A very small item I know but I would like to purchase one (2, 3, 4) if anyone has one to spare or would point me towards a commercial/private source.​
