I'm guessing that either your LR index file(s) are damaged or you deleted or moved the original (RAW?) image from the folder that LR thinks it is in, although one or more other original files are still in that folder. If you've moved stuff around on your computer, that is your problem. You have to put it back where Adobe expects it to be.
It's also possible that you have updated Windows and you haven't kept up with the updates from Adobe. If so, then download and install the latest LR updates.
If that isn't the problem, or you did that, my next step would be to go to the Adobe website and look through their FAQs. It could be a known problem that Adobe has a fix or work around for.
Failing that, try starting a new index and re-import everything and start anew.
Last, I'd uninstall LR, reboot your computer, then reinstall LR...and pointing to the new index folder/import.
Failing all the above, then go back to the Adobe website and contact their technical support.
