I saw this picture on itunes and was wondering to myself how on earth did they do that

Okay so lighting wise the image is taken fairly simply with a soft Rembrandt. What I want to know is how did they achieve that fantastic bokeh and lens flare to the side of the image. Is that in post?
Help me deconstruct this picture tpf!

Update, so I tried just the circular paintbrush in PS and splattered some bokeh, blurred it a little and played with some overlay settings but still nothing like that image

I would say that it's all post. The bright bokeh spots and the soft corners. Also, I don't think the light source is all that soft. Looks like possible a beauty dish. It's still pretty punchy, but has a slightly soft, but quick transition area.
