It's all photoshop.
I can do it with one click in Alienskin Exposure 2 for photoshop. It's called "glamour focus".
There's lots of ways to do it...none of which I'd be able to help you with...ahah sorry.

Okay, well I know it's done in photoshop... I just wanted to know which specific settings were altered to make it look like that... Thanks for letting me know the name of it... now I can google it!
In Photoshop it can be done by use of Filter>Blur>Gaussian Blur.
