But really a LOT of that is post processing.
 
A skilled Photoshop retoucher. Garbz is right; those are as much illustrations as they are photos.
 
so this is mainly photoshop work?

Yes, actually the best tutorial I ever saw for this was on kelbytraining.com

It's all about bringing out the highlights (brightening them) and darkening the shadows, the results are incredible and with a bit of skin touch up to remove the blemishes it appears perfect.

Now if I remember correctly the process goes as follows -
Load in image, clean up the skin, blemishes etc
Create new layer, set mode to soft light and check the box "Fill with soft light neutral colour gray"
Then use set the colour to white and use a soft brush and paint in the highlights to bring out the shape of the face and other areas.
Change the colour to black and paint in the shadows making them appear much "richer"
play with the opacity until it looks good
and walah!
