Was that scanned from print or from negative?
If from print, it is possible that the frame was really underexposed and the printer pushed the exposure on the frame as far as it will go. This resulted in a very flat and grainy photo.
If you were using ISO 100 film with your camera set to ISO 400, the same can occur. Your Pentax automatically sets the ISO read from the coded film canister.... so I"m not so sure incorrect ISO setting was the culprit (unless it was overridden)
