Where do we order "Order Forms"?

We are trying to find a supplier for those envelope order forms you usually get when you take team photos...One or both sides are printed with the products and the bottom has an envelope attached. You tear the perferations (sp?), seal the envelope and turn it in. Does anyone know where we can get them made?

Off the top of my head, if you are in the U.S., how about Office Max or Kinko's ?

I was on the Staples.com website this morning. They had a huge custom print section where they can do all kids of custom envelopes/flyers/whatever else. You might find what you are looking for there. Good luck with the search!
