Flash submit button

I am trying to get a submit buttong to work on a form for my unit's web site. https://www.8mcd.usmc.mil/ This is the main link, the current form does not work, though. https://www.8mcd.usmc.mil/PAB/EDWS Bio.pdf. Any ideas / advice / tips and tricks would be greatly appriciated. Also let me know what yall think of the site. Thanks in advance for all of yall's help.
