How do I re-create this pic?

I want to re-create this picture, but do not know what type of camera was used. Is this a 35mm, Holga, DSLR? Can I shoot this with a DSLR? If so, what type of settings am I looking at?

Looks like a flash mounted on the hotshoe (going by the shadows).
