He is writing, isn't he? And resting the left hand on something as not to smear what he already wrote? And he has written for a long long time ... alas! we shall never know what it is he wrote, unless you tell us.
He can't walk properly.
He needs crutches.
They are by his side.

That story is being told by a very well composed frame with the writing disappearing in the lefthand upper corner, I really like it!
