Nikon 500 f4 p manual focus lens with a TC-301 2x converter, D7200, F4 (8), ISO 64o, 1/800th. Not too much of a crop here at all. Steadiness is my enemy. Even with a sturdy tripod and mirror up, I need a little more support. I've heard of people using two tripods, one under the lens and one under the camera. Might work but it seems like it would be impossible to position the camera.

BTW, I have done a pretty good bit of testing, under controlled conditions, this lens loses zero sharpness with the TC-301.

Part of the problem when trying to photograph the moon is a roiling atmosphere here on Earth.

We want the moon as high in the sky as it can get at out location so we are shooting through the least amount of atmosphere possible.
Make sure the Moon is not over a building giving off heat, making that part of the atmosphere even more turbulent.

You don't need ISO 640 because the moon is bright. Use the base native ISO for your camera (ISO 100?) and a faster shutter.
The 2x TC isn't helping any either.
