With the title, you do get the sense. Without it, you would probably not, or at any rate the picture is much more open to interpretation. You might, you might not.
Think about how you could increase the sense of dread. Can you play with the color temperature or the tonal placement (I think there are things you could do here). Could you burn and dodge to focus attention on some specific element of the scene to increase the sense? (I think you could).
There's nothing particularly wrong with using a title to direct the experience of a picture, but you should be aware that this is what you're doing, and that the title is therefore an integral part of the piece.
