In general portraiture, there's no way to keep someone's hands and face in sharp focus if their hands are held out toward the camera. If the aperture is small enough and the camera is close enough, even a difference of centimeters will throw things out of focus. It also sounds like maybe your autofocus is picking up the closest object, so you have to decide on a better mode and make better focusing decisions. If you want the face in focus, put the focus point on the face and you might have to sacrifice some sharpness on objects that are much closer to the camera. If not, back off and zoom in and try to get a smaller aperture. That will compress objects and the distance will increase DOF.

Yep, I was using all auto focus points. Just learned by experiences.

"f-number" is the same as the "now serving" number in a bordello.
