Doing the actual technical work is easy with Photoshop. Being able to come up with the compositions takes a lot of talent and artistic ability.
You also need a large library of backgrounds and items to use.

You need a graphic designer to do something like this.

I read the complete details for doing this in bookstore yesterday. The artist had laid out the steps used. Total time to do about 3 hours. It's completely possible with PS. However, unless you are proficient with the basics of PS, and have access to a library of stock images, it would be extremely difficult.
