Online request for a photo - price and parameters?

Can others edit my Photos
Photos NOT OK to edit
Hi everyone

I got a flickr-mail from a guy (musician/composer) this week (and a second, more urgent follow up last night) asking me if he could use my below photo for the cover art for a choral piece that he will be self-publishing. Not sure if that means "online" or a CD cover, or what...

If I were to do so, I was considering telling him:

1. nonexclusive rights
2. my name must be used
3. my website address (probably blog, since I don't have a specific website yet) must be used
4. price ?? (75 bucks? I doubt I'd get much for it, and this would be maybe my first actual sale of a photo).

Is there anything else you could think of? I was planning to tell him that for that price I wouldn't give him a full sized file (i.e. I don't want him to be able to make a 24x 36 poster for himself). I also have that same photo taken with my P&S camera, and I may use that one instead because he can do less "damage" with a smaller file if he ends up sending it far and wide.

You need to ask him what size he needs for his project and some type of contract needs to be written up.
