It's basically a light stand with a boom on it.

Something like this: Adorama Boom Arm with Weight Bag and Light Stand LSB1720

It allows me to place the beauty dish directly above and in front of a subject to give a semi-harsh, dramatic light without having to worry about the stand being in the way. It also allows for more range of motion. On a normal light stand, you can't tilt the beauty dish down very much before hitting the stand itself.
