I went through this process as a parent customer about 2 years ago. Here are my recollections:
We had a team shot and an individual shot. The photographers did the team shot exactly as you would expect: one row kneeling, one row standing. The individual shot was down on one knee holding the football (I can't remember if the helmet was in there somewhere). Each kid had a numbered card that they held up and were shot with before the individual shot (seems like a great way to keep the kids straight).
We had to make the buy commitment before the shots were taken - there was a form that had to be handed in with a check on the day pictures were taken. I think this makes much more sense that trying to coordinate some sort of photo review and sale process after the fact, as some of the previous responders seem to be implying.
Unfortunately, I can't remember what we paid but the price was based on many options. Basically, the form had a menu of options from wallets and keychain thingies up to 8x10s including those cute trading cards with the kid "stats". There was no CD or other electronic option.
I think some of the prices getting mentioned here are insane. Don't expect many people to pay $50 for a CD. The norm for kid activity photos like this is prints in my experience. A CD only forces people to have to go off and spend more money, assuming they're willing to pay such a high price for the CD in the first place, not to mention the fact that some people would probably need an explanation of the various crop versions of the shots.
I'm thinking we paid somewhere in the $20-$30 range for several prints of varying sizes. If I had to pay $50, I would decline.
