I think the biggest problem is revealed in the original post: "soft light layer at 30% opacity (this seems to even out skin tones) dodged the eyes at 12 exposure, and unsharpened mask just the irises".

She doesn't NEED to "have her skin tones evened out". She's like 10 years old, not fifty. And the sharpening of the eyes, along with an eye dodging; those two things totally,totally work AGAINST the evened-out, bland-ified skin tones. The result is that you have bright, sharp, sparkly eyeballs, and a flat, yellow-tinged face, where there appears to be plastic skin. You've taken a shot of a lovely young girl, and ruined her face by "evening out" everything, then cranking up the eyes. That's my editing feedback.

Look at her cheek structure, and her dimples; there is a slight shadow there, but the soft light layer has destroyed the delicate shape and the subtle shading of her skin. It all looks plastic.
