Help fitting the Opteka wrist strap...

I purchased this strap and so far haven't been very successful in fitting it to my hand, as it leaves my trigger finger and thumb in pretty awkward position.

How tight should it be and do you use it with the padded section over your knuckles or over the back of your hand? What about reversing the padded section?

(I am using this on the d3000)
