It's done in post. You shoot the scene, then prop the model on something and shoot them. Then in post you clip out the model and their shadow and drop them onto the "clean" shot of the scene. Or you can have your model "jump" while trying to hold a neutral pose and get the shot of them in the air.

No way. A crane rig like that is too impractical. Most people doing these shots just use a stool and photoshop. I doubt the majority of people doing this have a crane in their homes.
