kaleidoscope effect on photos?

this is my first post so sorry if i say/do something wrong !! but i was wondering how i could recreate this photo ?

i was hoping to take a photo of my friends lips and im curious if this effect is done by editing in photoshop or if i need a certain lens to create it. thanks !!

First thing you did wrong was to use a photo you didn't take yourself or get the rights to use.

Umm, those filters still available ?
I used to sell those in the olden days ... never knew why anyone really wanted to pay a lot of money for these special effect filters.
Kendo used to make kits of these types of filters

I'm going to guess that particular image is a photoshop effect BUT it is possible to do this with a real filter. The Cokin 203 filter does that (except you get the 13 images... one in the center and 12 more copies of the image all the way around the sides.)

Get a prism and shoot through it to create mirror effects
