It's very tough to have your subjects be accessorized with jewelry on a shoot(espcially with all strobs/flash), I try to avoid it as much as possible as refelctions will be difficult to control. Being that she's laying down, the earing isn't complimenting. Nothing beats an upright model when wearing accessories, they're just designed for that.

As far as the lighting, she has a hotspot on the shoulder there. Her face can use a little umph of exposure and complimentary shadows.

Nah, just use the sync shutter speed of 1/200 - that usually kills the ambient, especially if you are shooting flash. If you have doubts, set your exposure for whatever f/stop you want to use, set the shutter to the sync speed and take a test pic without the flash. If you get "black", then there's no chance of ambient contribution to your photo - otherwise you'll see how much (and where) the lit-up areas are.

Somebody once told me the delta or difference needed to be 7 f/stops between ambient and flash in order to be absolutely safe against ambient light showing up...

What would the easiest way to find your delta in a situation? Just set your F#/Iso and raise your shutterspeed untill you have an all black exposure? Then set your flash accordingly?
