Permission to reproduce form?

Anyone know where I can find a good permission to reproduce form? Or any permission to reproduce form at all? Ive searched google but couldn't find anything that was clear cut. If you have your own you'd be willing to share I wouldnt mind either Im just looking for something that people can give to the copyright enforcers at photo labs.

In the past, I've simply typed a note stating that "This document may be considered as permission to reproduce my image as follows: "

Then I state the intended use, such as to make a specified number of copies, limited to the one occasion, and describe the image.

Sign and date.

Thanks Pete, instead of "this image" Ive said "this disk" - do you think that would be a problem?

I'm sure it will be fine. The vendor just wants some assurance that it's OK to do the work. The only time there would be a problem is if you should challenge them for going beyond your expressed release... using it for more occasions than allowed, or using it for a different client.

and... I just Googled "photo copyright release form" and got a few sites that might apply.
