Preferred Vendor List

How do you usually get on the preferred vendor lists of hotels, restaurants, etc. for wedding photography? Who do you approach?

Try the Catering Director or Banquet Manager.

I'm not a wedding photographer but I have an ex-Banquet Manger in my family. In her hotel, she was the person that made all those decisions. Each hotel or hotel chain may be different or have different policies, though.

In any case, I would make sure you've done a few weddings in the particular hotel and can show off your work there before making that request.

You can also find out if the hotel has a house band or house florist and get in good with them. Maybe do some free or low cost promo shots for those vendors in exchange for a good word with the hotel people.
