I never knew that. So you just cut out a large shape and then make sure your subject is inside and then crop the edges?The ones that you usually see, come from specular highlights (small bright areas). The shape of them reflect the shape of the aperture in your lens. Higher quality lenses often have more aperture blades, thus a rounder aperture opening, and some say...a more pleasing bokeh.
You can also shape the 'bulls eyes' by cutting out a shape in a card/paper and holding it in front of the lens.
