It's the sync cord that goes to the speed flash mounted on the bracket.

no guesses on the lens, though I would hazard something like a 50mm f1.4 or some such - maybe even a wider 30mm lens.
As for the rig - looks like a pro range body in size coupled with a flash bracket holding a speedlite and with the sync cord wrapped round the lens; The hotshoe with the adaptor is under her left hand

as for that big dangly thing I think its some form of flail on a chain (fine chain of course) for celebraties to beat fans off with

The pro bodies dont have grips. theyre so pro that they have TWO shutter release buttons. I'd be just as scared as that guy if that scary woman took my camera away from me.
