I use the whiten teeth brush with the auto mask box checked and then desaturate. It gives it a more natural look.

I'll have to try that tonight. I haven't messed with the teeth brush in LR4.

Nope. I rarely use preset things. I used the adjustment brush tool and masked off the whites of eyes in various gradients, then adjusted to taste with exposure, shadows, etc.
