Model Sign off forms

has anyone got any model sign off forms? just something basic that I can ammend myself. I do not want to write one of these from scratch.

thanks for this. it really was a mamoth reply, and shed a lot of light. i have heard also about getting releases for architecture shots too, and for brand names.

so taking a picture of a model outside the kremlin holding a cõcä cölä can would be quite hard to sell to anyone it seems. too much effort ((

i have a template now that I can use for an upcoming photoshoot, so thanks.
