Two more little tips for this type of shot for subject direction.....

1) Have them to try and pull their ears back and up. This reduces crows feet slightly.

2) Have them push their tounge to the top of their mouth and suck. This slightly helps with turkey neck.

Big Mike is spot on with butterfly lighting.

Right..?? I definitely tried this, and the tongue thing too and either I'm just lacking standard abilities or both don't work for me. lol

For the butterfly shadow placement, the light needs to be right over the lens axis,

I'd amend that to say that it should be right over the vertical face axis (nose axis)...because you can still have butterfly lighting if the pose isn't straight on to the camera, can't you?
